How many databases & instances of Reality can a system have

The recommend maximum for live commercial use is 14 databases in each of 10 running instances of Reality = 140.

In a standard installation you can have a maximum of 16 databases.
However, when configured for resilience, Transaction Logging and FailSafe (twin systems), this is restricted to 14. Therefore, the recommend maximum for live commercial use is 14.

Database Isolation:
NPS recommends using Database Isolation when many "live" production databases need to be deployed. A number of completely isolated "instances" of Reality installations can be defined, which both protects databases from operational and security issues and increases the number of database to 14 to 16 per instance for a system.

As you can define up to an additional 9 instances of Reality, as well as the base instance, this provides for up to 160 databases per system, 140 when resilience is deployed.

If required, each instance can also run a different version of Reality, which is useful during the rollout of Reality upgrades or participation in the trials of new releases.
